The main office/Hospice Inpatient Care Center is located just West of Nova Road off Dunlawton Ave. in Port Orange. From I-95, go East on Dunlawton Ave. (exit #85 at Port
Orange). Go approximately two miles East on Dunlawton. Turn left on Woodbriar Trail. Follow campus signs to Hospice of Volusia/Flagler.
The Hospice of Volusia/Flagler House in New Smyrna Beach is located at 1020 Claudia St. From US 1
(go approximately 1 mile south of the NSB Airport or one-half mile north of Canal St.), turn onto Eleanor Ave. Turn right on Claudia. Go ¼ mile, House is on your left.
Ormond Beach Office: located at 555 W. Granada Blvd., Suite H-8. From US1
go North to Granada Blvd. Turn left (West) onto Granada Blvd. A half block past (West) of Orchard St., turn right (North) into the Boulevard Executive Office Center. The office is in the first building on the right, door number 8.
West Volusia DeLand Office: located at 1250 S. Spring Garden (S.R. 15-A). I-4 to Exit 116 (for Deland/Lake Helen). Turn left on Orange Camp Road. Follow that to Woodland
Boulevard and turn right. Go to Spring Garden (S.R. 15-A) and turn left. Continue on same road for about a mile. Hospice of Volusia/Flaglers West Volusia office and BeginAgain Grief Center are in the Spring Garden
Plaza (1250 Spring Garden) just past the chiropractic business.
Orange City Resale Shop: located at 876 Saxon Blvd. Take I-4 West to exit 111 toward Orange City. Merge onto Saxon Blvd. Turn left at the fourth traffic light onto Enterprise Road. Take
the second entrance on the right into the Market Place shopping center. The shop is the first shop on the left.
Palm Coast Resale Shop in Palm Coast is located at 140 Palm Coast Parkway NE, near Bealls Outlet.
Hospice of Volusia/Flagler Resale Shop in South Daytona: From US 92, head south on Nova Road. Travel about one mile past the Beville Road intersection. The Shop is on the east side
of South Nova Road in the Landmark Business Park.
Flagler-Palm Coast Office:
Located at 7 Florida Park Dr., Suite A. Take I-95 North to exit 289 exiting right off the ramp onto Palm Coast Parkway. Turn left at Florida Park Drive. Cross over Palm Coast Parkway. Turn right into the Pavilion which is 7 Florida Park Drive.
Hospice of Volusia/Flagler Pineapple Palm Hospice House is located at 93 White Hall Drive in Palm Coast. From I-95:
Take Exit # 284 and turn left on S.R. 100. Pass the Flagler/Palm Coast High School and turn right on Belle Terre Parkway. Travel north for 3.1 miles and turn left on Whippoorwill Drive. Travel west about 1 mile to White Feather Lane and turn right. Follow White Feather Lane to the stop sign at White Hall Drive, and Pineapple Palm House will be immediately on your right.
From U.S. 1:
Turn onto White View Parkway, then left on Whippoorwill Drive. Travel to White Feather Lane and turn right. Follow White Feather Lane to the stop sign at White Hall Drive, and Pineapple Palm House will be immediately on your right.
From Palm Coast Parkway:
Travel south on Belle Terre Parkway and turn right onto White View Parkway. Turn right onto Whippoorwill Drive, go to White Feather Lane and turn right again. Follow White Feather Lane to the stop sign with White Hall Drive, and Pineapple Palm House is immediately on your right.
