Define Home Health Care As the term implies, home health care is simply health care services provided in a patient's home. Services are available 24-hours a day and can
be medically related or provide personal care. Medically related care includes skilled nursing services, home health aides and rehabilitation. Personal care includes homemakers, companions, sitters and
care management services. Home care staff can also teach family members what they need to know to help them care for their loved one.
What does it mean to be "homebound" and why it is important? Whether or not a patient meets the definition of "homebound" can play a role in determining the extent home health services
will be covered by Medicare, Medicaid and private insurance. Private pay services are always an option. A patient is homebound if he or she experiences a normal inability to leave home. The patient's physical
condition and physical limitations are such that it would be considerable and taxing effort for that patient to leave home.
Determining homebound status depends on the illness or limitations of the patient. The need for supportive devices or assistance alone does not necessarily render the patient homebound. The need for supportive devices and/or assistance in conjunction with physical limitations is a consideration when determining homebound status. Homebound status is not affected by regular absences from the home when the reason to leave is to receive medical treatment, attending church services on a regular basis or receiving treatments in an adult care program.
Home Health Team Our registered nurses and licensed practical nurses provide a complete range of nursing care in the home. Your nurse is the vital link between
you and your doctor. He or she will keep your doctor informed of your progress with the program of care designed especially for you. Our team includes many health care professionals including: Skilled Nurse
- provides treatments and procedures requiring specialized skills. Assesses vital signs and observes symptoms. Instructs patients and family members regarding care at home. Home Health Aide
- provides personal care assisting patients with such things as bathing, dressing, oral hygiene, and light housekeeping. Physical Therapist - provides exercise programs, and training on
equipment and prosthetic devices. Speech Therapist - teaches patients and family members in speech and language improvement. Occupational Therapist
-helps physically/emotionally ill patients restore independence. Medical Social Worker - Assesses the social and economic needs of patients to provide assistance with recovery.
Patients with Cancer Patients with cancer can receive a variety of oncology services in the home from certified oncology nurses.
These services include chemotherapy (Halifax Home Health only), I.V. therapy, patient and family education, and other oncology nursing care. You also have access to the many resources of the Regional Oncology Centers at Bert Fish Medical Center in New Smyrna Beach, and Halifax Medical Center in Daytona Beach and Ormond Beach.
Personal Care Services When skilled services are not required, personal care services may be the answer. Personal care services include companions, sitters,
housekeeping and meal preparation.
Rehabilitation We offer a wide variety of rehabilitative services that can be offered in the comfort of your own home. Our team includes
registered physical, speech and occupational therapists. Each can provide in-home therapies that assist in the maintenance of functions and, in many cases, an increase in independence.
Solutions for Independent Seniors Geriatric Care Management provides seniors who are in need of support, assistance and monitoring to maintain and/or enhance their health and independence. Who can
benefit? Seniors who have no family in the local area, who live alone or with a frail spouse, or who suffer from chronic physical and/or emotional problems. Pick from a menu of services to customize a
plan that meets the patient's needs.
All plans include a comprehensive, individualized assessment by a certified care manager (CMC) and provide a holistic approach to addressing independence and quality of life issues.
Wound Care Program (Halifax Home Health Only) Nursing specialists provide a thorough wound assessment with recommendations for dressing and wound management, along with complete nutritional assessment with
education and instruction for patient and family.
Additional services such as special support surfaces like mattresses or cushions, education in proper transfers and patient positioning are also available - all in the comfort of home.
