Halifax Managed Healthcare Assistance Program (HMHA) The Halifax Medical Center Taxing District provides support to individuals deemed indigent or medically needy
utilizing the Federal Poverty Guidelines. To be eligible, an individual must be a resident of the Halifax Medical Center’s taxing District.
A patient may apply at one of two HMHA locations within 45 days
from the date of service: Halifax Medical Center HMHA Office – first floor off main lobby, past Human Resources 303 N. Clyde Morris Blvd. Daytona Beach, FL 32114

Hours: Monday, Tuesday, Thursday and Friday, 8 a.m. – 1 p.m. and 2-3 p.m. (Closed for lunch from 1-2 p.m.) Phone: 386-239-6182 Clients are served on a first come, first served basis
HMHA Checklist
Important: It is your responsibility to provide the following minimum requirements at the time of your visit. Additional information may be required.
Identification Requirements:
- Florida ID or Florida drivers license AND Social Security card
- If you are married, you will need the identification on your spouse
- Dependent children – U.S. Birth Certificates and Social Security cards for each
Residency Requirements:
- Applicant must reside in Halifax Hospital Medical Center Taxing District and be permanent resident for 90 days.
- TWO of the following documents must be provided:
- Property Tax Bill (This indicates your taxing district)
- Lease or Rent Receipt
- Utility Bill
- Letter of Support if applicant is living with another party

Eligibility Patient Assistance Liaison Service (EPALS)
EPALS is an on-site partner providing assistance for the application process with Victims Comp*, SSI and Medicaid as long as you have an existing account with Halifax Medical Center within 90 days from the date of
service. *Other requirements apply for Victims Comp

Medicaid Three basic groups are eligible for Medicaid:
- SSI recipients
- Minor Children and families, and
- Aged, blind and disabled people, including people needing institutional care.
An individual must meet specific eligibility requirements in order to get Medicaid.
Each program has specific income and asset limits that must be met.
Persons may be eligible for full or limited benefits, depending on the program.
