Q. How do I go about dealing with the Business Office in person?

A.   For your convenience we have a Customer Service representative located in the
       Admission Office of Halifax Medical Center, 303 N. Clyde Morris Blvd., Daytona
       Beach. The Admission Office is located on the first floor across from the information
       desk, just inside the main lobby.

Q. How long does my account remain with the hospital before being referred to a
     collection agency?

A.   When your account is referred to a collection agency depends on many factors,
       including the account balance, the type of insurance, and whether or not you took
       steps to satisfy the debt upon the receipt of your statement(s). On average, an
       account is sent to a collection agency 120 days from the time the balance is
       determined to be the patient’s/guarantor’s responsibility.

Q. Where can I send my payment when my account is placed with a collection
     agency?

A.   You can send your payment to the address on the collection agency’s statement.  If you
       do not have that information you may forward your payment to:

Q. How do I obtain my medical records?

A.   Please contact the Health Information Management (HIM) Dept., Release of Information
       Section:

       Mail: Halifax Medical Center, 303 N. Clyde Morris Blvd., Daytona Beach, Florida 32114
       Phone:  386-947-2923 or 2924 (hours: Monday – Friday, 8 a.m. to 4:30 p.m.)
       Fax:      386-947-2925
   * NOTE: To obtain your x-ray films, contact the Radiology Department at 386-254-4047

Please complete the Authorization to Release Medical Records
You can fax, mail, or bring in the completed Request Form to the address shown above.

What you need to know to help the process work smoothly:

  • Please allow 3 - 7 days to complete your request.
  • Be sure to include your phone number on the authorization, in the event we need to contact you for further information.
  • For confidentiality/security reasons, The HIM Dept. can fax only to your physician or another medical facility.
  • If you choose to pick up your records, please call the HIM Dept. before coming in, to be sure the copies are ready.
  • When picking up your records, you will need to show a picture I.D.
  • The HIM Dept. will release your records only to you or the person you named on the authorization under "disclose to".
  • The authorization must be signed by the patient (in the event of a minor, the parent must sign) or by the guarantor of the estate or legal guardian, with proof of relationship.
  • We suggest you request only what you need, as there is a fee for the photocopies.
