Child & Adolescent Emergency Consent
Health Information
Except for life-threatening emergencies, hospitals must receive
parental permission to begin treatment of a sick or injured child. This Emergency Consent program at Halifax Medical Center is important when your child is brought to the Emergency Department, Halifax Urgent Care
(Port Orange and Ormond Beach) by a babysitter, neighbor or relative and you cannot be reached. A pre-authorized consent form is kept on file in the Emergency Department and allows us to treat your child in the
event of an emergency. To receive this form and take advantage of this free service, simply call 386-254-4042.

Health Information
A full-service, nine-bed Pediatric Emergency Department is staffed 24 hours a
day with pediatricians and support staff specially trained in pediatric emergency medicine. For information about the Pediatric Emergency Department, call 386-254-4181.
