NOTICE OF MEETING
The Board of Directors of Halifax Fish Community Health
will meet, in conjunction with the Board of Directors of Bert Fish Medical Center and the Board of Commissioners of Halifax Medical Center, on Tuesday, November 6, 2001, 4:00 p.m., at Bert Fish Medical Center, 401 Palmetto Avenue, New Smyrna Beach, Florida. If a person decides to appeal any decision made by the Committee with respect to any matter considered at this meeting, he or she will need a record of the proceedings, and for such purpose, he or she may need to ensure that a verbatim record of the proceedings is made, which record includes the evidence upon which the appeal is to be based.
