Healthy Start Care Coordinators Program
A partnership between Healthy Communities, Halifax Keech Health Center, and the Healthy Start Coalition of Volusia and Flagler counties was formally established in June 2000 to better serve the healthcare needs of
families living in the 32114 zip code in Daytona Beach.
Through the hire of two full time care coordinators, eligible families are clients of Healthy Start and are managed by the care coordinators in order to enhance treatment compliance and outcomes, effectively track
the needs of Healthy Start families and also to provide follow-up care, ensuring the needs of the families and children are met.
The care coordinators spend the majority of time on-site at Halifax Keech Health Center with patients three years old and under. In addition, the care coordinators will provide home health assistance as needed.
