NOTICE OF MEETING
The Board of Commissioners of the Southeast
Volusia Hospital District and the Board of Directors
of Bert Fish Medical Center will meet on
Tuesday, June 10, 2003, 4:00 p.m. at Bert Fish
Medical Center Board Room, 401 Palmetto Street, New
Smyrna Beach, Florida. If a person decides
to appeal any decision made by the Board with
respect to any matter considered at this meeting, he
or she will need a record of the proceedings and for
such purpose he or she may need to insure that a
verbatim record of the proceedings is made which
record includes the evidence upon which the appeal
is to be based.

Commissioners of Halifax Medical Center, and the

p.m., at 5111 South Ridgewood Avenue, Suite 101,
Port Orange, Florida.

Quality & Service Committee will meet on
Wednesday, May 21, 2003, 8:00 a.m., at 5111 South

Note that a portion of the meeting shall be closed
pursuant to Florida Statute 395.0197 If a person
decides to appeal any decision made by the Committee
with respect to any matter considered at this
meeting, he or she will need a record of the
